Title: Medical Coder
Duration: Full Time
Location: Remote
• Accurately assigns diagnosis and procedure codes for facility and professional services for Ambulatory Procedure Visit (APV), Dental surgical procedures, Observation, Emergency Department (ED), outpatient ERSA, and Outpatient encounters IAW DHA completeness, productivity, and timeliness standards. Work may involve areas such as Laboratory, Radiology, and Dental services. Codes records with correct Ambulatory Payment Classifications (APCs); and Relative Value Units (RVUs) in order for the Center to receive correct reimbursement or workload credit. Performs necessary tasks within MHS GENESIS and other military coding systems to complete encounters. May be tasked with assisting with outpatient coding if available.
• Adheres to accepted coding practices, guidelines and conventions when choosing the most appropriate diagnosis, operation, procedure, ancillary, or E&M code to ensure ethical, accurate, and complete coding.
• Monitors ever-changing regulatory and policy requirements affecting coded information for the full spectrum of services provided.
• Maintains technical currency through continuing education and training opportunities.
• Reviews encounter and/or record documentation to identify inconsistencies, ambiguities, or discrepancies that may cause inaccurate coding, medico-legal re-percussions or impacts quality patient care. Identifies any problems with legibility, abbreviations, etc., and brings it to the provider’s attention. May perform assessments and examine records for proper sequence of documents, presence of authorized signatures, and sufficient data is documented that supports diagnosis, treatment administered, and results obtained.
• Develops and submits a written (electronic or hard copy) query IAW DHA guidelines to the provider to request clarification of provider documentation that is conflicting, ambiguous, or incomplete regarding any significant reportable condition or procedure. Monitors query submission, response times, and completion.
• Educates and provides feedback to providers and clinical staff to resolve documentation issues to support coding compliance. Assigns accurate codes to encounters based upon provider responses to queries and reports queries and responses IAW DHA guidance.
• Acts as a source of reference to medical staff that have questions, issues, or concerns related to coding. Responds to provider questions and provides examples of appropriate coding and documentation reference(s) to provide clarity and understanding. Based on contacts from the medical staff, identifies training opportunities and works with coding training personnel to focus on consistency and clarity of coding advice provided.
• Collaborates with Medical Coding Trainers in developing, delivering, and monitoring initial and annual coding training to providers and clinical staff by providing guidance to professional and technical staff in documentation requirements for coding. Responsible for assignment of accurate E&M, ICD, CPT and HCPCS codes and modifiers from medical record documentation into the Government computer systems.
• Supports DHA coding compliance by performing due diligence in ethically and appropriately researching and/or interpreting existing guidance, including seeking clarification from the Lead Medical Coder, supervisor, or DHA-MCPB. Performs administrative related tasks associated with medical records final reviews/audits and contacting various departments, services, or medical staff to obtain data needed to complete the records. Complies with DHA coding compliance requirements regarding training and reporting of potential violations.
The Inpatient Medical Coder will possess post-high school education or training from ONE of the following:
• An Associate’ degree or higher in Health Information Management or Healthcare Administration, healthcare related major, or biological science; OR
• A University certificate in medical coding; OR
• At least 30 semester hours of University/College credit that includes relevant coursework such as anatomy/physiology, medical terminology, health information management, and/or pharmacology; OR
• Successful completion of an American Academy of Professional Coders (AAPC) or American Health Information Management Association (AHIMA) coding certification preparation course for professional services or facility coding that includes medical terminology, anatomy and physiology, health information management concepts, and pharmacology; OR
• Successful completion of a training course beyond apprentice level for medical technicians, hospital corpsmen, medical service specialists, or hospital training, obtained in a training program given by the Armed Forces or the U.S. Maritime Service under close medical and professional supervision.
• Possess a minimum of five (5) years of medical coding and/or auditing experience in two (2) or more medical, surgical, and ancillary specialties within the past 10 years;
• OR a minimum of three (3) years of medical coding or auditing experience if that experience was in an MTF. A minimum of one (1) year of performance in the specialty is required to be qualifying.
This position requires possession of a current coding certification in good standing from EACH of the following categories:
• Professional Services Coding Certifications: ONE of the following recognized professional coding certifications: Registered Health Information Technician (RHIT); Registered Health Information Administrator (RHIA); Certified Professional Coder (CPC); or Certified Coding Specialist – Physician (CCS-P).
• Institutional (Facility) Coding Certifications: ONE of the following recognized institutional coding certifications: Registered Health Information Technician (RHIT); Registered Health Information Administrator (RHIA); Certified Outpatient Coder (COC), or Certified Coding Specialist (CCS). Other institutional coding certifications will be considered by the DHA-MCPB on a case-by-case basis.
• Advanced knowledge of the International Classification of Diseases, Clinical Modification (ICD-CM); Healthcare Common Procedure Coding System (HCPCS); and Current Procedural Terminology (CPT), as used in institutional and professional services medical coding.
• Advanced knowledge of reimbursement systems, including Prospective Payment System (PPS); Ambulatory Payment Classifications (APCs); and Resource-Based Relative Value Scale (RBRVS).
• Advanced knowledge and understanding of industry nomenclature; medical and procedural terminology; anatomy and physiology; pharmacology; and disease processes.
• Practical knowledge of medical specialties; medical diagnostic and therapeutic procedures; ancillary services (includes, but is not limited to: Laboratory, Dental, Occupational Therapy, Physical Therapy, and Radiology); and revenue cycle management concepts related to medical coding.
• Practical knowledge and understanding of Government rules and regulations regarding medical coding, reimbursement guidelines, and healthcare fraud; commercial reimbursement guidelines and policies; coding audit principles and concepts, and potential areas of risk for fraud and abuse. Includes, but not limited to: The Federal Register, Center for Medicare, and Medicaid Services (CMS) Local Coverage Determinations and National Coverage Determinations (LCD and NCD), National Correct Coding Initiative (NCCI) guidance, manual, and edits, Internet-Only Manuals (IOMs), and HHSOIG publications and reports.
• Practical knowledge of clinical documentation improvement and continuous process improvement processes.

SUMMARY:

QUALIFICATIONS:
- Successful completion of an accredited Aerospace Medicine Residency and served as an Air Force Chief of Aerospace is preferred but physicians having served in a flight medicine billet for at least 1 year may be acceptable if approved by the Chief of Aerospace Medicine. Other DOD and civilian trained Flight Surgeons may be considered but require Chief of Aerospace Medicine approval.
- Current board certification by one of the following boards: Aerospace Medicine Board, the American Board of Family Medicine, American College of Osteopathic Family Physicians, American Board of Preventive Medicine, or board certified by the American Osteopathic Association through the American Osteopathic Board of Preventive Medicine or General Medical Officers with at least 1 year of flight medicine clinic experience assigned as a 48X duty AFSC or other branch equivalent will be considered. License cannot be under investigation nor have any adverse action pending from a State Board or national licensing/certification agency
- Possess a current certification in Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) and Pediatric Life Support (PALS) in accordance with 355 MDG policies.
- Working knowledge of computer operations and proficiency in the use of basic word processing, data entry and automated medical records.
RESPONSIBILITIES:
Primarily responsible for assessing, diagnosing, treatment planning, and behavioral health treatment.
- Provide appropriate military specific documentation such as (but not limited to) narrative medical summaries for Medical Evaluation Boards and medical profile paperwork.

- Focus on primary health care requirements. Flight medicine providers may be used to sign Return-To-Fly 1042s, waivers, or fly preventive health assessments only if they meet the qualification criteria listed in the Civilian Flight Medicine Physician Performance Work Statement and approved by the Air Force Medical Operations Agency (AFMOA/SGPF). Flight medicine providers will not fly (as part of their duties), respond to in-flight emergencies, after-hours call or Hazardous Materials (HAZMAT). Flight medicine providers will not participate in exercises beyond the scope of medical care, will not deploy, and will not be utilized as Squadron Medical.

LOCATION: Naval Medical Center - Portsmouth, Virginia
QUALIFICATIONS:
- A fully qualified typist with a minimum of 50 WPM is required.
- General medical ethics, telephone etiquette, and excellent communication and customer service skills.
- General office administrative and clerical skills to perform receptionist duties and answer telephones.
- Ability to communicate effectively, both orally and in writing.
- High school diploma or General Educational Development (GED) equivalency.
- Medical terminology desired.
- At least 6 months of experience in office setting.
- Current certification in Basic Life Support (BLS).
- The work is primarily sedentary. However, there may be some physical demands.

- Creates appointment schedules and templates in patient appointment computer system.
- Conducts end-of-day process at close of business and resolves any delinquent or pending appointments in computer system.
In addition to the job description provided Lead and Assistant Lead Medical Office Clerks who provide services to the Radiology Clinic must:
- Be a liaison with the Government supervisors (Government supervisors will not direct or control MSS Personnel). MSS Personnel shall be the on-site representative assisting with employee work schedules, work planning and organization, work assignment and review, and supervisor personnel functions for MSS Personnel.
- Manage and Supervise Avosys Personnel assigned to Radiology department, in coordination with Avosys Corporate Supervisor.
- Independently coordinate and execute Radiology department Schedules and Coverage operations to assign duties, as required.

SUMMARY:
Provides administrative and clinical review and administrative support for the DoD’s implementation of the American College of Surgeons (ACS) National Surgical Quality Improvement Program (NSQIP) program. This includes but is not limited to, clinical screening, data compilation documentation and entry into the DoD NSQIP data base of all eligible patient cases within the medical treatment facility. The SCNR also works closely with the members of the Department of Surgery to identify opportunities for clinical quality improvement. The program collects data on cases, such as demographics, surgical profile, preoperative, intraoperative and post operative mortality and morbidity outcomes for patients undergoing surgical procedures in both inpatient and outpatient settings.
QUALIFICATIONS:
- Knowledge of professional nursing principles and procedures in surgery, either the operating room, intensive care unit or inpatient surgical ward.
- Must have knowledge of accreditation standards, privacy and confidentiality requirements, such as, The Joint Commission and HIPAA.
- Computer skills and abilities. Must be comfortable using Microsoft Office programs (Word, Excel, PowerPoint and Access), Outlook (e-mail), and Internet.

EDUCATION

Assist in routine care and treatment of patients, to include flying and special operational duty personnel, when required. Organize the medical environment, perform and participate in support activities for outpatient care situations, including contingency operations and disasters.
QUALIFICATIONS:
- Successful completion of all education and training requirements leading to and graduated from an accredited national, state or military equivalent school for Practical Nursing.
- Practiced as a Licensed Practical Nurse (LPN) performing hands-on patient care in the outpatient clinical setting for at least 12 months within the past 24 months. In lieu of 12 months experience within the last 24 months, the Government will consider candidates that worked at a Military Treatment Facility (MTF) within the past 24 months in order to become a LPN.
- Possess and maintain a current certification in Basic Life Support (BLS) in accordance with 355 MDG policies. Current certification in Advanced Cardiac Life Support (ACLS) and Pediatric Life Support (PALS) is highly desirable.
- Possess and maintain throughout course of contract, a current, active, valid, unrestricted license (with no limitations, stipulations or pending adverse actions) to practice as a Licensed Practical Nurse (LPN) from a US jurisdiction. License must be recognized and accepted by the State of practice in accordance with governing regulations. License cannot be under investigation nor have any adverse action pending from a State Board or national licensing/certification agency

- Experience performing care and treatment of patients; operating and maintaining therapeutic equipment; properly administering oral and parenteral medications; conducting paraprofessional portions of physical examinations; or assisting in medical treatment of patients in accordance with established procedures and protocols, within the LPN scope of practice.
- Must have a professional attitude and speak, write and read in the English language.
RESPONSIBILITIES include, but not limited to:
- Serve as a Practical Nurse performing patient care and nursing administrative duties for outpatients in accordance with established policies, procedures, protocols, and guidelines.
- Monitor and record age appropriate physiological measurements.
- Orient new patients to the clinic/hospital environment.
- Assist with patient check-ins, admissions, discharges, and transfers as directed.
- Provide individualized patient teaching on appropriate topics to include preventative healthcare and health maintenance.
- Prepare and position patients for diagnostic testing, examinations, treatments, and clinical procedures.

LOCATION: Remote work from home.
HOURS: Monday thru Friday 8 hour shift with core hours between 8-2 CST 9-3 EST, no overtime is allowed.
SUMMARY:
Perform clinical reviews of Medicare Part A and Part B claims for Medical Review, Redeterminations/Appeals (Appeals), and Prior Authorization requests (collectively, the “Services”) in accordance with CMS (Centers for Medicare & Medicaid Services) requirements.
QUALIFICATIONS:
- Minimum of two (2) years’ clinical experience

RESPONSIBILITIES: include, but not limited to:
- Clinical review of services:
- Review medical record documentation within CMS timeliness parameters (i.e., 20 days from receipt of the medical record for pre-payment reviews and 50 days from receipt of the medical record for post payment reviews).
- Utilize the applicable Medicare policies (i.e., Local Coverage Determinations, National Coverage Determinations, Internet-Only Manual (IOM) citations, inpatient tools, etc.) to ensure the services comply with all Medicare regulations and documentation requirements.
- Review documentation for medical necessity per guidelines outlined in the Social Security Act 1862(a)(1).
- Ensure that all documentation includes a valid signature consistent with the signature requirements.
- Documentation of rationale for processing decisions:
- Provide a claim sample of three (3) claims to Companies via established protocols and timeliness parameters (i.e., 18 days from receipt of the medical record for pre-payment reviews and 48 days from receipt of the medical record for post payment reviews) for quality review prior to finalization of documentation of reason for payment, reduction, or denial of service to ensure accuracy of claim decision making.
- Companies will review the three-claim sample for accuracy of claim decision and will make and return decisions to the MRS within 24 hours or less.
- Complete the documentation of the reason for payment, reduction, or denial of service for all claims on an electronic decision template to be provided by Companies. This rationale must be in sentence format so that it may be inserted directly into the response to the provider, must be clear and well-written, and contain sufficient information to educate the providers on how the review decision was made.
- Return documented decision electronically to Companies via established protocols and timeliness parameters (i.e., 20 days from receipt of the medical record for pre-payment reviews and 50 days from receipt of the medical record for payment reviews).
- Complete the review results letter in the Companies’ letter writing system within 35 days from receipt of the medical record for pre-payment reviews and no later than 60 days from receipt of the medical record for post payment reviews.
- Document all case activity in Companies’ provider tracking system on the day the activity occurs.
- Complete one-on-one provider education (i.e., webinar, conference call, etc.) within 30 days of sending out review results letter.
- Respond to provider inquiries related to case and/or claims throughout the course (i.e. in 24 hours or less) of review.
- If additional clinical guidance is required, complete the Contractor Medical Director (“CMD”) assistance form, track response, and update review accordingly.
- Conduct telephone development for missing or additional records for easily curable errors.
- Notate date of receipt of additional documentation received in the Companies’ provider tracking system.
- Upon request by Companies, initiate or participate in provider teaching activities, creating written teaching material, providing one-on-one education or education to a group as a result of an MR review.
- If fraud activity is suspected, immediately complete initial referral packet for external entity referral and return the packet to the Companies.
- Complete referrals to Companies’ provider outreach and education (“POE”) area in provider tracking system for cases that have a moderate or major error rate.
- Lead and Alternate Lead will participate in all monthly departmental training and meetings, and all Staff will participate as requested.
- Submit all cases for review and approval for quality and closure of cases.
